Cigna's Dental Plan is a managed care dental program which operates
through a network of participating dentists.

Highlights of the plan feature:
. No annual deductibles.
. No annual or lifetime maximums.
. No charge for most preventive and diagnostic services.
. Co-payments for fillings, bridges, dentures, crowns and orthodontic
care.
. No claim forms.
. Dependent coverage to age 19; full time student coverage to age 25.

Quarterly premiums:
Single: $103.28
Two Family Members: $210.52
Full Family: $310.24
